New Graduate Student Information Guide
Immunization
Health Center
Campus Center (1C), Room 112
(718) 982-3045
Proof of Measles, Mumps and Rubella immunization is required by law before registration for all students born on or after January 1, 1957, who are enrolling for 6 or more credits. Meningococcal meningitis vaccination is also required but you may opt to sign the meningitis refusal form instead of receiving the vaccination. Immunization records can be obtained from previous schools attended and childhood vaccine records. Immunization forms should be given to the Health Center before your advisement and registration session. You may download the immunization forms below:
If you previously attended CSI and submitted this information, it is not necessary to do so again.
