Registrar
State law requires immunization against measles, mumps, rubella (MMR) and meningitis prior to enrollment. Students may opt to sign a meningitis refusal form in lieu of the vaccination.
All students who are enrolling for 6 or more credits or equated credits must have proof of immunizations on file in the College Health Center one week prior to registration. Students born prior to January 1, 1957, are not required to comply with the MMR requirement. For first time students one vaccination of measles, mumps and rubella is acceptable for partial compliance. If these criteria are not met the student will be excluded from classes.
This rule applies to all new, currently enrolled and readmitted students regardless of entry date into the College.
Affected students failing to submit the form will be limited to a program of 5 credits or less.
Immunization forms are available at the Registrar's Office, Room 2A-110B and the College Health Center, Room 1C-112.
You can download your immunization forms at the registrar important forms page.
