Monday, Tuesday, Friday 9:00 a.m. - 5:00 p.m., Wednesday and Thursday 9:00 a.m. - 8:00 p.m.
Can I fax my immunization and or medical records?
Yes, the fax machine is on twenty four hours a day.

Can I mail my immunization records to the Health Center?
Yes, Please put "Attention: Health Center, 1C-112" on the envelope and keep a copy of the records mailed to us.
If I had the measles, mumps, and rubella immunizations (MMR) in childhood do I have to get immunized again?
Measles, Mumps, and Rubella Immunization given on or after your first birthday (measles on or after January 1, 1968, and mumps and rubella on or after January 1, 1969) are good for life. However, NYS law requires two proofs of measles inoculations, so you may have to get another vaccination or submit positive titers.
Where can I obtain my immunization records?
Records can be obtained from the following sources:
- An official school immunization record. Please note: If you are a recent NYC high school graduate and you apply through UAPC your immunization record is automatically forwarded to the Health Center. If you do not fit that category then it is your responsibility to contact the appropriate source and request your immunization record.
- An official childhood baby book immunization record.
- An official medical documented immunization record.
- Lab results that prove positive immune blood titers.
Does the Medical Certificate for Participation in PED 190 have to be completed before I register?
This form must be completed (stamped, dated and signed) by a physician before registering for PED 190. If you are not registering for the PED 190 course you do not have to submit the form.
Where can I be immunized for Measles, Mumps, and Rubella (MMR)?
The Health Center offers students free MMR vaccine. Call or come in for an appointment.
How can I get a copy of my CSI immunization records?
Health records are confidential. You can request a copy of your records in writing. The letter must state that you give the College of Staten Island permission to release your immunization and/or medical records. Then give the name, address, or fax number of the agency you want to receive your records. The letter must also be signed and include your student identification number. The letter can be faxed to us or you can go to the Health Center and fill out a Medical Record Request Form.
How long does it take to get a copy of my CSI health records?
Most immunization records are available on computer and can be printed out immediately. Microfilmed records can take up to two weeks to retrieve. There may be some instances where records are no longer available.
What steps must I follow in order to obtain a medical withdrawal?
You need medical documentation that includes date(s) of illness and diagnosis. Prior semesters require the instructor's permission. The Instructor's Release Form is obtained in the Registrar's Office. Only WU, INC, and FIN grades can be changed. Medical withdrawals are subject to the regular College refund policy. See the schedule of classes for more details.
Does CSI offer students discounted health and dental insurance?
Yes, GHI and several other carriers offer students health and dental insurance at a discounted rate. Applications are available in the Health Center at the beginning of the Spring and Fall semesters until the enrollment period expires.
What are the requirements for GHI insurance?
You need to be registered for 6 matriculated credits or more per semester.
Does GHI also offer Family benefits?
Yes. There are plans available with and without prescription drugs.
Where do I get further insurance information?
- By logging on to www.ghi.com, or calling GHI at (212) 501-4444 Monday through Friday between 9:00 am and 4:45 PM. A GHI representative visits the campus each semester. Call the Health Center for date and time. You can get additional GHI information by going to www.csi.cuny.edu and going to the Health Services section.

How and when can I get an HIV test?
A community outreach educator from
Community Healthaction of Staten Island
provides HIV counseling, education and HIV testing at the CSI Health Center. It is free, confidential, and oral (mouth swab, no blood). Results are available in 20 minutes. A current photo ID is required. Call or stop by for an appointment.
