Double Telepresence Robot Loan Agreement Form

The Double is a Telepresence Robot that can be used to assist faculty, staff, and students in various initiatives, made available through the Office of Technology Services.

Terms and Conditions Governing Use of the Double
  1. The Double must be booked 2 weeks in advance of event, through the HelpDesk (718.982.4357)
  2. The user must have training on the features and maneuvering of the Double, which must be completed at least 3 days before the event.
  3. While in their possession, user is responsible for all parts of the Double, which include the following:
    1. Stand with the lateral stability base 
    2. Audio kit
    3. iPad Air 2 
    4. Power Adaptor
    5. Camera Kit 
    6. Carrying Case
  4. While in physical possession of the Double, user can never leave it unattended.
  5. While a user is connecting to the Double from off campus, they must wait for OITS personnel to pick up the Double, before dropping connection.
  6. The Double can never be used outdoors, as it will cause damage to the base.
  7. The Double can never be taken off campus. If needed at St. George, OITS staff will deliver the Double on site.
  8. User will be emailed a link to connect to the Double for the duration of the requested event.
  9. Anyone connecting to the Double from off campus is responsible for obtaining their own web-cam.
  10. The user and/or department will be responsible for covering cost of repairs of any damage to the Double while it is in their possession.
